Just a little understatement;) If it is not an extrnal body, then I think it must be mechanical failure of the piston. Rare if not unheard of on the stock MIV piston. At one bar and 65- 70 revs a second that rod had plenty of time to smash the hell out of what ever was left at the top of the bore once it broke free. I would guess this: We then installed an AEM peak and hold driver They are notorius of braking and melting a cylinder. Same thing happened to my friend.
