Iirc the DPA questionaire can never be used in any form of meeting i.e.disciplinary, recruitment or promotion or more an accurate statement 'it's not meant to' and should only be used for statistical purposes. You should contact your HR department and they are legally bound to advise you how this information is/will be used. What does DPA stand for? Any information your company holds on you, you can ask for to see it and also how it will be used. Also see Freedom of Information Act, both may halp you if you wish to know more. Only after they'd set aside a nice warm padded windowless cell room for you and a snug fitting straight jacket coat, all of your own I'm with Chris W on this one, find out why they are asking and if you can refuse to complete some or even all of it. I'm a bit confused by one of the questions though - 'Sexual Orientation'. So it says you can put 'same sex' 'opposite sex' or 'same sex and opposite sex'. My question is, what does that have to do with your job and working environment? Surely if you like to sleep with men and women that's your concern? Is this now standard proceedure? i think its to protect managers for wasting time coming on to attractive lesbians. We have something similar at work and in all seriousness i think its to help with possible future sexual harrassment discrimination litigation, and possibly also to help protect any minors in the workplace. Don't fill it out if you don't want to - no doubt they'll default to 'opposite sex' unless told otherwise, but I doubt anyone actually thinks the question is important enough to get worked up about.
